INFORMATION FOR PATIENTS

PRIOR THE VISIT


It will be helpful to write down the significant facts about the history of your problem.
The following questionnaire may help to know what to look for:

  • When and how did your problem begin?
  • What you were able to do prior to your injury (thing at home or recreational activities)?
  • How often do your symptoms repeat?
  • How is the pain / symptom changing over time (intensity and quality)?
  • What makes things better or worse?

Prior to your first visit, we invite you to complete and sign the intake patient form. Be sure to bring it to your first visit. The form below is a PDF file, which could be filled out in the browser, printed, and signed by you personally.
Please note that if you do not complete these forms prior to your first visit, you will be required to fill them out in our office.

MEDICATION LIST


It is helpful if you bring a list of your medications and any surgeries or procedures you have had in the past.


WHAT TO WEAR ON THE DATE


Be prepared to move around a lot during your first physical therapy session, as well as to expose the problematic area to be treated. With that in mind:

  • choose clothing that is easy to move around in
  • something that will not be too restrictive if it needs to be moved out (or taken off) if the way

As an example: If you have shoulder pain, wear a shirt that allows access to your shoulder from the front, back and side. Shorts should be worn if you have hip pain, knee pain, or ankle pain. You can bring a change of clothes for this purpose if needed. Please don’t forget to wear comfortable shoes, you might be required to take on and off your shoes multiple time throughout the session.


YOUR FIRST SESSION


Your first session is called Initial Evaluation. What should you expect during this session?

  • We will investigate and uncover as much as possible about your condition, your previous level of function, and how your current situation is affecting your life and functional levels.
  • We will do a physical examination to take specific measurements to examine and pinpoint some impairments that may be causing your problem.
  • We will put together a personalised treatment plan for you.

DEDICATED PARKING SPOTS

iPark (Imperial Parking Systems) is available. It is located in the Century building. It offers a discount for our visitors when parked for up to 2 hrs.
(Please ask receptionist to stamp your parking ticket prior returning to your car).


FOR PARKING IN THE AREA

Allow yourself on average 10 to 15 min for locating a parking spot within a blocks distance.
Street cleaning days for parking purposes are Monday and Wednesday mornings.


ARRIVE ON TIME


Please arrive on time. we cherish your time and strive to utilize it as effectively as possible.

  • A normal visit will last in average 45 to 60 minutes.
  • For an initial evaluation allow yourself between 1 and 1.5 hours.


WORKING HOURS

We are open during the working week. The indicated open hours may vary. Please call prior the appointment to confirm.
